
The HCBS-DD Waiver provides residential services for individuals with intellectual and developmental disabilities (IDD) in a host home, family care, or other approved residential setting that promotes independence, health, safety, and community inclusion. Services are provided by qualified caregivers who offer individualized supports and 24-hour supervision based on the person’s assessed needs and service plan. Daily supports may include personal hygiene, medication administration, meal preparation, laundry, behavioral support, coping and social skills development, community integration, transportation, supervision, and activities of daily living.

Children’s Extensive Supports (CES) services for children with intellectual and developmental disabilities (IDD) provide individualized, family-centered supports that promote development, independence, community inclusion, and overall quality of life. Services are designed to meet the unique strengths and needs of each child and family while supporting participation in home and community settings. Services may include respite, community connector services, massage therapy, extraordinary cleaning, skill development, and other supportive services. Homemaker and personal care services will be provided under Community First Choice (CFC) under our pending Class B licensure approval. Coming Very Soon! We are able to provide Homemaker ONLY under the CES Waiver.

The Children’s Habilitation Residential Program (CHRP) Waiver supports children and youth with intellectual and developmental disabilities (IDD) and high behavioral or medical needs by helping them remain safe and supported in their homes and communities while reducing the need for institutional or out-of-home placement. Services are available for eligible youth over 18 and under the age of 20 and are designed to promote stability, independence, and family support through individualized, person-centered care. Services may include 24-hour residential habilitation, respite, massage therapy, community connector services, supported community connections, extraordinary cleaning, and other supportive services. Homemaker and personal care services will be provided under our pending Class B licensure approval.

Supported Living Services (SLS) assist individuals with intellectual and developmental disabilities (IDD) in living as independently as possible in their own home, apartment, family home, or other community-based setting. The HCBS-SLS Waiver is designed to supplement natural supports and community resources through individualized, person-centered services that promote independence, community inclusion, and daily living skills. Individuals receiving SLS services do not require continuous 24-hour supervision. Services may include respite, massage therapy, supported community connections, prevocational services, homemaker services, extraordinary cleaning, and other supportive services. Homemaker and personal care services may also be provided under Community First Choice (CFC) through our pending Class B licensure approval.

Personal Care and Homemaker services for individuals with intellectual and developmental disabilities (IDD) are designed to support health, safety, independence, and daily living within the home and community. Homemaker services may include light housekeeping, meal planning and preparation, laundry, dishwashing, changing bed linens, bathroom and kitchen cleaning, vacuuming, dusting, sweeping, and mopping. Personal Care services may include assistance with bathing, dressing, grooming, toileting, mobility, eating, medication reminders, and other activities of daily living based on the individual’s assessed needs. These services are offered under Community First Choice (CFC), with Homemaker services currently available and Personal Care services pending approval under our Class B licensure. Coming soon!

Prevocational services for individuals with intellectual and developmental disabilities (IDD) are designed to build the skills, work habits, and social competencies needed to support successful transition into competitive integrated employment or other meaningful vocational opportunities. These services focus on increasing independence, confidence, and readiness for future employment through structured skill development and community-based learning experiences. Services may include developing and maintaining appropriate work behaviors, learning interview skills, improving communication, practicing time management, strengthening problem-solving and task completion skills, and building teamwork abilities. The overall goal is to support each individual in achieving greater vocational independence and successful long-term employment outcomes.

Supported Community Connections (SCC) provides individuals with intellectual and developmental disabilities (IDD) meaningful opportunities to engage in social, recreational, and community-based activities that promote inclusion, independence, and overall well-being. These services are designed to support individuals in building relationships, developing social skills, and participating in activities that reflect their personal interests, preferences, and goals. SCC may include participation in community clubs, recreational programs, educational or enrichment classes, sports and fitness activities, volunteer opportunities, cultural and arts events, faith-based activities, and other community outings. Services also focus on increasing confidence, communication skills, and community navigation abilities while reducing social isolation and promoting long-term community engagement and connection.

Respite care for individuals with intellectual and developmental disabilities (IDD) is a supportive service designed to provide temporary relief to primary caregivers while ensuring the individual continues to receive safe, structured, and person-centered care. This service helps prevent caregiver burnout, supports family stability, and promotes overall well-being for both the caregiver and the individual receiving services. Respite care may be provided in the home or in approved community settings and can range from a few hours to longer periods of care depending on assessed needs and service authorization. During respite services, individuals may receive assistance with activities of daily living, supervision, social engagement, and participation in age-appropriate recreational or community activities, all while maintaining consistency with their individualized care plan and support goals.

Massage therapy for individuals with intellectual and developmental disabilities (IDD) is a supportive therapeutic service designed to promote relaxation, improve overall well-being, and enhance quality of life. When tailored to individual needs, massage techniques can provide calming sensory input, reduce stress and anxiety, and support emotional regulation. This service may also help address muscle tension, physical discomfort, and pain management needs while encouraging a greater sense of body awareness and relaxation. In addition to physical benefits, massage therapy can support improved mood, reduced behavioral challenges related to stress, and increased overall comfort. Sessions are individualized to ensure a safe, respectful, and person-centered experience that aligns with each participant’s preferences and support plan goals.
